At CJC Services Unlimited, Inc., we evaluate each potential client and carefully create a customized plan.
Initial Consultation
Our Geriatric Care Consultants will evaluate the Client’s home situation/condition if suitable for home care and will determine the services a Client needs and might need in a case to case scenario such as:
Emotional State
Environmental Factors & Safety
Daily Needs
Consultation and evaluation of client needs can take place in Client’s home, assisted living facility or medical unit.
Choosing a Caregiver/Companion
Based on the Client’s initial consultation results, CJC Services will do a careful selection process as follows:
Determine which caregiver/companion reflects the Client’s interest, personality and any specific preferences.
CJC Services will do the best to meet and match Client needs in every aspect.
Developing a Personal Care Plan
A personally tailored care plan will be developed by CJC Services consultant to meet the Client’s needs based on the following:

CJC Services caregiver/companion helps the client achieve maximum independence while being in the comfort of their home, assisted living facility or medical unit.
Evaluation of Care Plan
Our geriatric care consultant will come to the comfort of our client’s home to evaluate the effectivity of the developed personal care plan.
Re-visit and redevelop a personalized care plan that suits our Client’s preferences.
Inputs from Family and Medical Professional providers for our Client’s are all incorporated into the care plan development.
